UNIQUE STUDENT IDENTIFIERS- THE RTO CANNOT ISSUE A CERTIFICATE WITHOUT HAVING OBTAINED A USI FOR THE STUDENT

We are now required to ascertain the following from all students before enrolling

  1. Identification
  2. Proof of residency, this is to ensure that all students have the applicable visa rights to attend a training course in Australia

What's required?

  • A copy of your current Australian passport will fulfil both requirements
  • If you have not got a current Australian passport we require the following two pieces of identification
    • Copy of your current driver's license
    • Copy of your current medicare card

Please note if you cannot or do not provide this information we are unable to let you attend the training centre and the admin cost for enrolling and completing the online component will be deducted from your course fees before a refund is given. (if Applicable)

Privacy Statement, Third Party Disclosure and Declaration
Under the Data Provision Requirements 2012, the RTO is required to collect personal information about you and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER). Your personal information (including the personal information contained on this application form and your training activity data) may be used or disclosed by the RTO for statistical, regulatory and research purposes. The RTO may disclose your personal information for these purposes to third parties, including;

  • School – if you are a secondary student undertaking VET, including a school-based apprenticeship or traineeship;
  • Employer – if you are enrolled in training paid by your employer;
  • Commonwealth and State or Territory government departments and authorised agencies, NCVER; Organisations conducting student surveys; and
  • Researchers

Personal information disclosed to NCVER may be used or disclosed for the following purposes;

  • Issuing a VET Statement of Attainment or VET Qualification, and populating Authenticated VET Transcripts;
  • Facilitating statistics and research relating to education, including surveys and data linkage;
  • pre-populating RTO student enrolment forms;
  • Understanding how the VET market operates, for policy, workforce planning and consumer information; and
  • Administering VET, including program administration, regulation, monitoring and evaluation.

You may receive an NCVER student survey which may be administered by a government department or NCVER employee, agent or third party contractor or other authorised agencies. Please note you may opt out of the survey at the time of being contacted. NCVER will collect, hold, use and disclose your personal information in accordance with the Privacy Act 1988 (Cth), the National VET Data Policy and all NCVER policies and protocols (including those published on NCVER’s website at www.ncver.edu.au) Personal information is collected from you for the purpose of obtaining and verifying student related details. It is used by government departments for the planning, provision and reporting of educational and vocational training programs as authorised by the Privacy Act 1988 and the Skilling Australia’s Workforce Act 2005 and related State and Commonwealth Acts and Regulations. Information will be provided where required to Job Active Networks and their referral agents if applicable. Personal information will be managed in accordance with the RTOs P-039 Privacy & Personal Information Policy and may be accessed by the individual to whom it is related by a written request to studentenquiries@ash.com.au
